The purpose of this perpetual survey is to help DFPS plan for the anticipated workload associated with provider's T3C Credential Applications. By responding to this survey, the provider is in no way obligated to apply for the selected Service Package(s) or Add-On Service(s) and/or within the anticipated time frames. Please consider the Provider's plans for either Initial or Full Credentialing, from December 2024 through August 2027.
The Department would like to hear from as many providers as possible by the end of January 2025, and then at any time after that if your plans change. Having this up-to-date information in advance will help to ensure efficiency of the Credentialing process.
The estimated time to complete this survey is 15 minutes per permit.
